There is no option regarding medical certificate as it has to be made by government doctors only as a specified by the Medical Association of India. As you have got your medical certificate by a private doctor, it won't be entertained at the time of counselling or seat

Hello ! 

I guess you have some missconception. JEE main is an examination for Engineering field. You can get into colleges which provide engineering courses. If you want to do medical then you should have given exams like NEET. I guess you have tagged the wrong exam.
