There won't be any chance to gain free seat in Karnataka Medical Colleges but yes there would be some concession in the fee structure. You also require to give NEET in kannada if you want to gain admission in Government Medical college of Karnataka.

Well any student who clears the minimum qualifying marks announced by NTA is eligible to get a seat in medical courses all over the India.
