Well dear,getting a government college with a score of 500 is quite difficult,you may apply for private colleges,because telling exactly whether you  can end in a good college or not will be quite difficult as it depends upon a variety of factors.

Overall rank means the rank you got is amongst all the candidates in exams inculding the unreserved category or reserved category aspirants.

Category rank means the rank you got is of amongst the aspirant of that specific category appeared in the exam. Category rank is for the EWS,

In order to get a seat in a medical college in Karnataka through NEET exam you need to aim for a little bit high score i.e around 440-460 marks *if you are a general category candidate). 

As there are 5,617 seats available  for medical college and around 2,180

The highest mark scored was 691 out of 720 with an approximate 10% fall in eligibility cutoff last year . The  tamil nadu NEET CUTOFF for the all india quota in 2017 was 566 for UR , 572 For OBC and 442 for Sc the .The Tamil nadu

Well for you to get government colleges in Karnataka is not possible because it is not your domicile state.

According to your marks, chances seem to be less of getting admission in government medical colleges. You can try to get a seat in private colleges and Deemed Universities.
