The fall of rank in the second round of NEET counselling can not be predicted, as it only depennds upon the availability of vacant seats after first round of counselling. For some colleges , whose all seats got filled we can expect the admissions to be closed for the second round of counselling. While , in some colleges whose almost seats are vacant , you can even expect a rank difference of 2k. So , it is all a play of availability of seats.
To know the actual difference , you have to wait till the second round of counselling.
