You can't get into IIT''s on the basis of Diploma score only. Yes you can.... but you have to give JEE (MAINS & ADVANCE). But it will be lengthy and time killing. If you are doing polytechnic, then you direct get into lateral in state university / college, direct to 2nd year.

As you mention your percentile in your diploma but you didn't mention the branch what we should talk here and also about the course in which you want to get admission so kindly mention these detail...
If you talk about Mtech so for MTech admission in IIT candidate have to score around 55% for general category as mentioned that you are PWD category so yes you are eligible to to apply for IIT..
