I want to apply for btech cap rounds of cet by both JEE as well as cet scores, So is it necessary to fill the form separately ? or It's only to be mentioned in one application form ?

You have not mentioned for which state entrance exam you are talking about. But yes, it is necessory to register yourself separately if you want to appear n both the cousellings, either from state common engineering entrance or through the state CAP round counselling.
