For you to be an eligible candidate for NEET, you must have passed 10+2 or equivalent with Physics, Chemistry, Biology/Biotechnology and English as core subjects from a recognized board.
NEET eligibility marks in class 12 to be considered are aggregate marks only for PCB subjects
UR - 50%,
OBC/SC/ST - 40%,
PWD - 45%

As indicated in your question, you have already completed Class XII given that you already appeared for NEET once. Now the Code you will select depends on which Board you have passed it from.
If you completed your Class 12 from CBSE, ICSE or any State Board, then you should select Code 2.
But if you completed your Class 12 as a Pre-University Course (PUC) student/Pre-degree Course student/Junior College student , ie. your Intermediate/Pre-degree Examination in science of an Indian University/Board, then you should fill Code 3. The PUC qualifying exam takes place in very few states like Karnataka. Hence the applicable code will most likely be 2 in your case.
