in direct admission process for board toppers ,if first rank holder refuses seat ,can it is given to second ranker for bitsat?

I don't think so. It's not that BITS goes to every board topper and offers them a seat. It has a provision by which board toppers and only the toppers can apply.
It's not a rank wise cutoff that if one refuses the next one gets it

Absolutely. Why will BITS go to toppers when the good crowd is after it.
But the question id for direct admission and the merit students get that and definitely for that there is a certain criteria which each university/institute follows.
